An organized routine starts with the right format
Managing several prescriptions can mean keeping track of different containers, refill dates, and deliveries. SortPak offers two packaging approaches so patients and caregivers can choose an experience that fits their routine with help from the pharmacy team.
SortPak and SyncPak share an emphasis on coordination and personal support. Their main difference is how medications are organized and presented at home.
SortPak: pouches organized by prescribed day and time
SortPak uses clearly labeled medication pouches organized around the prescribed day and time. The pouches are supplied in a dispenser, giving patients a practical way to keep their medication schedule organized.
Ask the pharmacy team which of your medications can be included and how other items will be supplied. Packaging should reflect your current prescriptions; contact your pharmacist when your treating clinician changes a medication or schedule.
SyncPak: coordinated medications in one delivery
SyncPak brings prescription bottles and non-pill medications together in an organized delivery box. The focus is on coordinating the medication shipment while retaining individual containers.
If you or a caregiver prefer bottles, or need support coordinating different medication formats, the team can discuss whether SyncPak fits your needs.
What happens when you get started?
The process begins with signup information. A patient care coordinator then contacts you to understand your medication needs, and the team coordinates prescription transfers with your current pharmacy.
Before your first shipment, use that conversation to ask about packaging, delivery arrangements, and any prescriptions that need special handling. Your pharmacist and prescribing clinician remain your sources for medication instructions.
