A medication routine that members can manage
For a health plan member managing several prescriptions, the difficulty often sits between the prescription and the next dose. Different refill dates, transportation needs, changes in therapy and uncertainty about what to take can make a daily routine harder to manage.
SortPak Compliance Packaging Solutions brings medication synchronization, organized packaging, home delivery and personal pharmacy support into a coordinated service. The goal is to help members maintain their prescribed routine while giving authorized health plan teams a clearer view of the medication journey.
Our mission is to make medication routines easier to manage. Our vision is a more connected relationship between the member, the pharmacy and the health plan, with timely information that supports follow-up.
SyncPak creates a coordinated starting point
Members can begin with SyncPak while our team reviews their current medication routine, coordinates prescription transfers and works toward a shared refill date for eligible medications. Prescriptions remain in their traditional containers. Eligible inhalers, ointments, sprays and other non-pill items can travel in the coordinated delivery box.
The aim is fewer separate refill arrangements and a predictable delivery cycle. Synchronization may require short fills, prescriber coordination or payer approval. Not every medication can share the same date, and refill timing must remain clinically appropriate.

Graduating to SortPak pouches when the member is ready
Once eligible prescriptions are aligned and the member chooses dose packaging, our team can transition suitable medications to SortPak pouches. Each pouch organizes the prescribed dose by day and time, presented through the SortPak dispenser.
Graduation is a personalized step, not a requirement for every member. Some medications need their original containers, and some members prefer SyncPak. The packaging choice should fit the prescriptions, the member’s abilities and the support available at home.
Packaging service and delivery carry no additional fee. Medication copays still apply. SortPak packets are not child-resistant and must stay out of children’s reach.

A focused contribution to medication quality measures
SortPak supports health plans working on medication adherence for cholesterol therapy with statins, hypertension therapy with renin-angiotensin system (RAS) antagonists, and eligible diabetes medications. Coordinated refills, delivery planning and pharmacist communication can help address interruptions in access to prescribed therapy.
PQA uses proportion of days covered (PDC) to assess medication availability from prescription claims. A threshold of 80% applies to the three established adherence measures, subject to each measure’s specifications. PDC reflects medication supply, not confirmation that a member took each dose.
Statin Use in Persons with Diabetes is a separate prescribing and dispensing measure. SortPak can work with the plan and prescribing clinician to review a potential statin-use gap, clinical appropriateness and applicable exclusions. Packaging alone does not resolve every quality measure.
CMS awards Star Ratings at the health-plan contract level. SortPak supports medication-related quality efforts without guaranteeing a particular Star Rating, clinical outcome or financial result.
SortPak Live brings the member journey into view
Authorized health plan teams can use SortPak Live to follow enrolled members and coordinate with the SortPak service team. Live operational PDC scores, refill gaps, gap days and adherence trends help teams focus their follow-up.
A gap becomes more useful when the team can see the context: the last available fill, the refill status, outreach notes, a pending prescription or a delivery issue. Teams can review the available information with SortPak, agree on the next step and follow the documented progress.
Operational PDC reflects available dispensing and claims data. A complete picture depends on timely data sharing, including fills outside SortPak, reversals and applicable measure adjustments. Portal updates follow receipt of the underlying data and do not replace the plan’s validated CMS reporting.

A shared workflow for the health plan and pharmacy
At implementation, SortPak and the plan align the eligible population, referral process, member outreach approach, data feeds and reporting expectations. Coverage, network participation and member choice guide enrollment.
Our team helps coordinate prescriptions, refill timing and delivery while providing pharmacist support for medication questions. Authorized care teams can use the shared information to prioritize follow-up and collaborate with the prescribing clinician when a barrier requires clinical input.
For program reviews, plans can agree on reporting for enrolled members, refill gaps, PDC trends and documented outreach. Where the necessary data is available, reporting can also distinguish new enrollments and referral sources. These expectations should be defined before launch.
The goal is continuity that the plan can follow
The dispenser gives the member a simpler way to organize eligible doses. SyncPak gives the member a coordinated starting point. SortPak Live helps the health plan stay connected to the work around refills and access.
Together, these services give health plans an adherence support model centered on the member’s daily life and a shared view of the next step.
