First, we need to verify some information before SortPak can submit a request to your insurer for your COVID-19 tests.
Limits and exclusions
You must be enrolled in an eligible health plan. Each person covered by an eligible plan may purchase up to 8 at-home COVID-19 tests per month.
Your order may come with an extra test if we are unable to supply you with a single test pack.
While you may be eligible for the maximum quantity of tests, inventory may be limited due to supply chain constraints.
